This policy covers Fuerza Performance Training, owned and operated by Marco Carrillo-Muñoz (B.S. Exercise & Rehabilitation Science, ACE-CPT). It explains what happens to the information you share through this website, and especially through the coaching application form.

What information this site collects

This site does not require an account, and it does not run advertising trackers. The main way information is collected is when you choose to fill out and submit the coaching application. That form asks for:

Your browser may also share basic technical details with the form processor (described below), such as the time of submission. The host that serves these web pages may keep standard server access logs.

Why this information is collected

The information is collected for one purpose: so Marco can read your application, decide whether Fuerza Performance Training is a good fit for what you are after, and respond to you. It is not sold, rented, or shared for marketing.

How form submissions are processed

The coaching application is processed by a third-party service called Formspree. When you submit the form, your answers are sent to Formspree, which delivers them to Marco and may store the submission on Formspree's systems so it can be retrieved and managed. In other words, your submission passes through and may be retained by Formspree in addition to reaching Marco directly. Formspree handles that data under its own terms, which you can read at formspree.io/legal/privacy-policy.

Who can access your submission

Your submission is read by Marco Carrillo-Muñoz. Formspree, as the processor, has technical access to the data it stores in order to provide the service. The email account that receives your submission is a standard email inbox, so the same realistic limitations that apply to any email apply here.

How your information is used

Your information is used to evaluate coaching fit, to reply to your application, and, if you decide to work with Fuerza Performance Training, to begin setting up your coaching. If we move forward, you will be asked separately for the detailed health and training history needed to program safely.

How long information is kept

Applications are kept while they are useful for following up and for managing the coaching relationship, and are removed when they are no longer needed for those purposes. Data held by Formspree is also subject to Formspree's own retention practices. If you would like your application removed sooner, you can ask (see below).

How to request a correction or deletion

You can ask to see what was submitted, correct it, or have it deleted. Email mac@fuerzaperformancetraining.com with your request and the name and email you used, and Marco will take care of it. Some information may need to be retained where there is a legitimate or legal reason to keep it.

Security

Reasonable steps are taken to protect the information you share, and Formspree provides its own safeguards as the processor. That said, no method of sending or storing information online can be promised to be perfectly secure, so please keep that in mind when deciding what to include.

Please do not submit medical information through the coaching application. The initial form is only to start a conversation. It is not the place for diagnoses, medication lists, or detailed medical history. If you become a client, that information is gathered later through a separate, appropriate process. Fuerza Performance Training is not a medical provider and this is not a HIPAA-covered form.
